1) Free advance directive forms for all 50 states, from AARP.org

 

AARP.org offers free advance directive forms (a.k.a. healthcare proxy, healthcare power of attorney, or living will) for all 50 states.  Among other things, the advance directive designates someone (your agent) to make healthcare decisions  when you are unable to act or communicate.  This is very important!!  A best practice is to fill out the form, sign, have a witness or notary also sign, then distribute the form to loved ones and your chosen agent.  Do this today if you don't already have one!
